TLDR; A great game about the difficulty of doing everyday tasks with severe mental disorder(s). Unfortunately it's pretty short, but the Developer already says that in the description. In my experience it's more like 15-20 minutes, but i am also a slow reader.

This review contains spoilers and is my interpretation of the story

The Walk to the Store.
In "Milk inside a bag of milk inside a bag of milk" we are a voice inside the head of a young girl which she made up to help her. This could be a sign of schizophrenia, but also could just be a coping mechanism to help her with everyday tasks. We are supposed to help her buy milk for her mother, while doing so, we encounter multiple problems which are associated with different mental disorders.
The very first thing we learn is that, the person we help wasn't outside for a long time, because she self-isolates herself. While going to the store to buy the milk she is already practicing the sentence which she needs to say to the cashier, these two things together is a sign of Social Anxiety. While she practices the sentence we try to motivate her while she is frustrated that the practicing is even necessary.
Next we tell her that she was walking with one foot on the pavement and with the other foot on the grass, she freezes immediately and asks us for how long she has done that. 50 steps on the pavement, 51 on the grass. This is a trigger for her, because the steps are not even and since another trigger is to walk with each foot on different ground, she can't just make another step forward, this is a sign of OCD and/or Autism, while neurotypical people would just take a step to the side we need to help her to get out of this situation. Unfortunately we phrase it as "step one" because it's the first step to fix the problem, she doesn't understand it right and thinks we are talking about the first step she walked on different grounds, taking words this literally is a sign of Autism. We rephrase it and she asks us again to rephrase it another time, so she will understand what we actually want from her.
We build up some pressure, that she needs to get to the shop to get the milk while the store closes soon, this actually helps her. People with Mental Disorders often pressure themselves to get things done instead of saying something positive to themselves self for motivation.

The Store.
While still practicing the sentence that we need to say to the cashier, a other person talks to us. O
The girl asks what the person wants. O
She only repeats the exact same question, and the other person responds exactly the same every time. O
This goes on so long that we, as her Imaginary Inner Voice, need to step in.
She tells us why she is acting so weird by is showing us an image which she has in mind which scares her. After that, she continues asking, "What?" to the other person, in which the person still responds in the exact same way. O
After this goes on for some longer, just repeats after him, after she did that the other person leaves.
I am not sure which Mental Disorder this sequence is supposed to represent, it feels like a mix of Social Anxiety and OCD which is represented here.

At this point, we remind her that she is in the store to buy milk, which triggers another conversation with her and us. Including the game naming sequence, where she thinks about how weird/funny/silly it is that she is carrying a bag of milk inside a bag.
Next up is the situation she trained for on the whole way to the store. The cash register. This sequence works out pretty weird, the cashier needs to remind her to put the bag of milk on the cash register. She stutters while talking to the cashier. and the cashier only responds with one word. and the girl doesn't understand why. We need to step in to resolve the situation.

The Walk Home
While on the walk home she tells us, that she loves the pyramidal structure of verbal construction. This could be due to autism.
She thinks about the uneven asphalt she is walking on, because she is so deep in her thoughts, she almost gets run over. She doesn't understand how someone would kill an innocent girl like her, even though it wasn't an attempt to kill her, she was just so deep in her thoughts that she didn't realize what is happening around her. This could be caused by multiple Mental Disorders, Autism, ADHD, OCD, Dissociative Disorder, but being this deep in thoughts could also happen without being Mentally Ill.
We have a deeper conversation with her, where we learn about her hallucinations, which are most likely caused by Schizophrenia.

She sits down on a nearby bench to talk a bit more to us before we reach home.
In the following we also have to type in answers instead of just clicking an answer from the options.
She tells us a lot about her mental health, that she tried so many medications that she barely feels the difference of the medications anymore and the effects of the medications becoming less effective. Another thing that she tells us is that she regrets something and that she doesn't want to say which diagnoses she got, because she wants to be seen as who she is and not as the mental disorder(s) she got. This is something a lot of people with diagnoses face, no matter if mental or physical, but the catch with mental disorders is that you don't see them and when you tell people that you got diagnosed with something a lot of people suddenly behave different around you than they have done before.
She tells us about her history, what happened in the past which changed her life, she is even showing us a picture. She tells us that she thinks, that other people are afraid of her.
In this part of the game the developer also does okay work in telling the viewer that you don't see from the outside if someone is struggling with a Mental Disorder or is even suicidal, even when you are very close to a person.
After the deeper conservation, she mentions that there are boundaries between us, the player, and her. So she removes the volored border around the text box. This, together with the fact that we had to write answers ourselves instead of clicking on a given option could be a sign that she accepts us as part of her, which could be due to schizophrenia or dissociative identity disorder (previously known as multiple personality disorder).
We tell her that she should finally go home, to bring the milk to her mother, since this was literally what she was supposed to do from the beginning.

While almost home, she tells us that she was scared of heights in the past, but she found practice to get less and less scared of it. Also she tells us about unpleasant side effects of her medicine.
Now we are home and face her mother. This is pretty short-lasting, but does a great job in showing that her mother is not the best/friendliest.

This is also the end of the game.


conclusion
This game is one of the best visual novels I've played, which tries to show the difficulties of everyday tasks with multiple mental disorders. The game got a unique art style, which definitely is not for everyone and is also the reason why I wasn't sure if I should even play it. The playtime is sadly pretty short, and I wish it would have a longer story, but the story is also advertised as short and if you are a slow reader you will most likely also take a bit longer than the advertised time.
If you want a short visual novel with a great story, I would recommend you to buy this game.
